As you know, every year we have to get you to complete UPDATE FORMS, so if you have an appointment for your first visit of the year, please go ahead, print out the PATIENT ANNUAL UPDATE FORMS, from the FORMS tab, complete them and bring them to your visit along with your Insurance cards. If you cannot get this done before your appointment, please come about 15 minutes early for your appointment. Thank you!
